Express Registration

1. Complete the following mandatory forms (required):

a) Participation Waiver Form (online here
b) Volunteer Waiver Form (online here)
c) DVMS Volunteer Interest Form (online here
d) TDAP Immunization Confirmation (7th Gr Only)
e) Please Note: Purchasing a Directory from the DVMS PTSA Store is not the same as choosing to “Opt-in” to be listed in the                    Directory, so please make sure to do that too! Click here to view the Opt-In/Out online form.


2. Click on the icons below to purchase or make voluntary donations to support your child's education (optional):

4. Sign up to volunteer at school (click here for volunteer clearance).

5. Attend the Registration Walkthrough Aug 7 from 9am to noon, or 4pm to 7pm in the DVMS Gym (optional, but highly recommended).

6. Complete the SRVUSD Annual Update (required).

7. Purchase DVMS Spiritwear.  The 2018 DVMS Spiritwear collection is now available to purchase for a limited time. Click here to order. The store closes at midnight on September 21, 2018. Orders cannot be placed after that date. (optional)

2018 TRAFFIX PASS SALES FOR MONTE VISTA HIGH SCHOOL & DIABLO VISTA MIDDLE SCHOOL The TRAFFIX bus program is entering its 9th year of service, providing safe, convenient and affordable transportation for students. The program has helped reduce the number of vehicle trips during school pick-ups and drop-offs and significantly reduces the carbon footprint in Danville and San Ramon every year.
